Employee giving is an opportunity to further engage with our ministry by making a donation to one of CommonSpirit Health’s 80+ fundraising entities in support of a program or service that you are passionate about. National office employees are encouraged to consider supporting one of the CommonSpirit Health Foundation’s priority initiatives.
Thank you for standing with your CommonSpirit Health colleagues and supporting our mission to create a healthier future for all—inspired by faith, driven by innovation, and powered by our humanity.
CommonSpirit Health Terms and Conditions
For payroll/PTO gifts I understand that:
- cash donations are subject to all applicable payroll taxes and will be reported as wages on my IRS W-2 in the calendar year in which the cash is donated.
- any ongoing cash donation election will be in effect until discontinuance is requested by me in writing or by online request.
- the named foundation will receive a copy of my form (N/A if you have named a CommonSpirit Health fund).
Please NOTE: For tax purposes, the date of a Payroll or PTO donation is the date that funds are deducted and donated from your paycheck. Donations submitted in December may not be fully processed for 1-2 pay periods and the donation may occur in the following calendar year. If you would like to ensure your donation is completed in the current tax year, please consider donating by credit card online before December 31.
Cash donations to the foundations and to any CommonSpirit Health fund, except any Employee Emergency Assistance Fund, are tax-deductible to the fullest extent allowed by law. The foundation and CommonSpirit Health do not provide goods and services in consideration for contributions to its employee charitable giving campaigns.
By continuing you are agreeing to all terms and conditions that have been provided.
