Healthier Communities
Powered by Humankindness
Caring for the whole person goes beyond the walls of the hospital. CommonSpirit is committed to offering
programs that address the factors that contribute to poor health such as the lack of good housing, healthy food,
safe transportation, and resources for aging patients. We’ve created Community Care Networks that help people
navigate the complex health care system and access available resources inside and outside the hospital.
Your support has funded pathways for families to access healthy food, find shelter and transportation, and has
provided resources so families don’t have to choose between seeing their doctor and paying their rent.
Community Centered Care with Connected Community Networks
Connected Community Network (CCN) seeks to create health equity in communities by bringing together multiple stakeholders to connect community resources to underserved populations in need of vital services. With sites in several locations, the networks aim to address social, economic and environmental determinants of health for our most vulnerable patients.
CCN promotes a no wrong door approach, enabling greater access to and accountability for needed nonclinical services. This model shifts power from large systems to trusted community partners.
CCN is one more way that CommonSpirit Health is living our mission and working together to strengthen health equity.
You can support programs like Connected Community Networks when you designate your online donation to the Healthier Communities Fund.
