Healthier Communities
Powered by Humankindness

CommonSpirit Health serves a broad mix of people in diverse communities across the country.

Through our population health programs and commitment to value-based care we seek to expand access to primary care, advocate for the poor, and transition to a coordinated care approach that builds healthier communities.

Community Centered Care with Connected Community Networks

Connected Community Network (CCN) seeks to create health equity in communities by bringing together multiple stakeholders to connect community resources to underserved populations in need of vital services. With sites in several locations, the networks aim to address social, economic and environmental determinants of health for our most vulnerable patients.

CCN promotes a no wrong door approach, enabling greater access to and accountability for needed nonclinical services. This model shifts power from large systems to trusted community partners.

CCN is one more way that CommonSpirit Health is living our mission and working together to strengthen health equity.
